Specialty care focus

Care may include support pathways for patients with higher-risk conditions such as CHF, COPD, diabetes-related needs, fall risk, post-hospital recovery, medication changes, and other clinically appropriate concerns.

Not a replacement for emergency care

Patients with urgent or life-threatening symptoms should call 911 or follow physician/emergency instructions. Home health can help monitor and communicate concerns, but it does not replace emergency medical services.
